Does Pilates Actually Help Chronic Back Pain?

Pilates reduced pain and disability more than a minimal intervention in chronic low back pain, though it was not superior to other structured exercise types.

Pilates, Chronic Pain

Chronic low back pain affects most adults at some point, and Pilates has become a popular exercise choice for managing it. This 2013 systematic review pooled results from ten randomised controlled trials testing the Pilates method against no treatment, minimal intervention, or other exercise types. Compared with a minimal intervention, Pilates produced a meaningful short-term reduction in both pain and disability. However, when tested head to head against other structured exercise programmes, Pilates showed no clear advantage; it simply performed as well as alternatives, not better. The review's authors rated the overall quality of the included trials as generally low to moderate, a limitation worth bearing in mind when weighing how confidently to recommend Pilates over any other supervised movement programme. For anyone managing persistent low back pain, this suggests Pilates is a reasonable, evidence-backed option among several, rather than a standout solution.
